Gabapentin is an anticonvulsant medication used in the management of peripheral neuropathic pains, postherpetic neuralgia, and partial-onset seizures. Gabapentin acts at the nerve terminals of neurons. When an action potential arrives at the nerve terminal, calcium channels are activated allowing influx of calcium ions, which stimulates the release of neurotransmitters by causing the exocytotic vesicle filled with neurotransmitters to fuse with the membrane. The release of excitatory neurotransmitters like glutamate contribute to neuropathic pain and seizures. The role of gabapentin is to bind to the α2δ-1 subunit of voltage-gated calcium channels, thereby inhibiting the voltage gated calcium channels. In this case, when an action potential arrives at the nerve terminal, calcium ion influx is prevented and the release of glutamate neurotransmitter does not occur.
